SHARE - Social Prescribing

Connects seniors who are Self referred to or referred by their family doctor/GP or other healthcare providers, to community activities and/or services that interest them and that promote physical activity, nutrition, social connection and independence. Social Prescribing is a way in which older adults take control of their own health and become more engaged in their communities.

Referral from a doctor or other healthcare provider is recommended.

Who qualifies: All 65+ living at home in Tri-Cities
